                                    Medical scribing is a process wherein a specially trained medical scribe records and documents a physician's patient encounters in real time. The scribe is present in the exam room with the physician and patient, and the physician dictates the patient's history and exam findings, as well as any treatment plan or diagnosis into a microphone.  

                               The scribe then records this information into the patient's electronic medical records. Medical scribes are typically medical students or health care professionals who are trained to accurately and efficiently record the patient's data and to help the physician stay on schedule. Main duties of a medical scribe includes :

1. Each shift pairs a scribe with a single doctor.

2. Scribes accompany the doctor while they question and examine the patient.

3. Scribes record the patient's medical history as it is given by the doctor, including details on the patient's current condition, a review of their systems, past medical and surgical histories, social and familial backgrounds, medications, and allergies. Scribes record the physician's actions during the physical examination, including any discoveries.

4. As instructed by the doctor, scribes record the outcomes of laboratory and radiographic studies.

5. Scribes accurately record when activities relating to patient care, such as doctor-to-doctor conversation, family communication, and re-examination of the patient, take place.

6. The doctor will evaluate the paperwork submitted by the scribe at the conclusion of the patient's visit, make any required changes, and sign the chart. Documenting the patient's encounter must be done in the doctor's office.

7. To assess patient status, delays, and any other care-related issues, the physician and the Scribe will conduct chart rounds.

8. The doctor, not the scribe, must give all instructions about patient care.

9. Scribes should transfer any requests for patient care including, but not limited to, delivering samples, taking calls, helping patients, contacting doctors to the appropriate physician or member of the nursing staff.
